When the documentation answers none of those questions, E11.319 is the workhorse: Type 2 diabetes with unspecified diabetic retinopathy without macular edema. It is the most common retinopathy code on claims for exactly that reason.

When the documentation is detailed, the series rewards it. Mild, moderate, and severe nonproliferative diabetic retinopathy (NPDR) and proliferative diabetic retinopathy each have their own subcategory. A sixth character reports whether macular edema is present, and a seventh character reports laterality: right eye, left eye, or bilateral. E11.52 is for Type 2 diabetes with diabetic peripheral angiopathy with gangrene. The gangrene is already built into E11.52. What the claim often needs is an L97.x code when an ulcer is present, coded to site, laterality, and depth.

E11.59 is for other circulatory complications, the catch-all for documented diabetic vascular involvement that is neither classic peripheral angiopathy nor gangrene, such as diabetic microangiopathy.

The diabetic link still matters here. If a patient’s PAD is attributed to smoking rather than diabetes, E11.51 is wrong. Code the diabetes and the PAD separately unless the provider connects them.

E11.6 Series: Skin and Ulcer Complications

This is a big one. Diabetes is a major cause of foot ulcers. The code E11.621 is for Type 2 diabetes with foot ulcer.

When you use this code, you must add code to specify the site of the ulcer. Use codes from the L97 family for this.

Example: a patient has a left heel ulcer with skin breakdown due to Type 2 diabetes. You would use E11.621 and L97.421. If the note only said left foot without naming the heel or midfoot, the site code would come from L97.5 instead, the subcategory for other parts of the foot.

The L97 code is doing 3 jobs at once: it reports the site (heel and midfoot codes live in L97.4, other parts of the foot in L97.5), the laterality (right or left), and the depth (limited to skin breakdown, fat layer exposed, necrosis of muscle, or necrosis of bone). The final character is the depth, so an ulcer code ending in 9 tells the payer the severity was not documented. That is a documentation gap worth querying, because depth drives both clinical urgency and reimbursement.

E11.622 covers Type 2 diabetes with other skin ulcers, for diabetic ulcers that are not on the foot. It carries the same instruction: add the site code from L97. x or L98.4x.

And one trap: pressure ulcers are not diabetic ulcers. If the wound is documented as a pressure injury, the site code comes from L89.x with its staging system, not L97.x. Using L97 for a pressure ulcer is one of the most common wound-coding errors on diabetic charts.
